Guidelines For Key Requests,
Adopted 10/19/04
The collaborative use agreement between the Fairbanks North Star Borough School District (FNSBSD) and the University of Alaska Fairbanks/Tanana Valley Campus (TVC) in management of Hutchinson Institute of Technology notes the importance of maintaining building security while ensuring appropriate access to faculty, staff, students, and the general public.
Consistent with that agreement, this memo outlines TVC guidelines and procedures for requesting access (e.g. keys/electronic key fobs) to the building. This agreement has been developed in consultation with TVC faculty and staff as well as the Hutchison High School principal and will be reviewed periodically to monitor any security concerns and to ensure that legitimate needs for TVC community access are met.
The TVC Director will continue to be the first point of contact for any requests for new or revised access permission by TVC faculty or staff. This ensures that we maintain consistency in how access issues are addressed. It also insures that we have regular and consistent communication with FNSBSD personnel and that we monitor access and security issues carefully.
Access requests for regular TVC faculty and staff:
To request access for a regular TVC faculty or staff member, the program/department coordinator must do the following:
- Submit a completed UAF Facilities Key Authorization Request Form to the TVC Director’s Office for consideration.
- TVC authorized key requests will be forwarded to the FNSBSD Hutchison High School Principal’s Office for consideration.
- Approved key requests: TVC’s on-site administrative assistant will contact the appropriate person/program when the key is available for pick-up through the principal’s office.
- At the time of key pick-up the employee will be required to complete and sign a FNSBSD Key Authorization Form. Please be prepared to provide a photo ID card to the FNSBSD at the time of key issue.
- FNSBSD will return the approved key request form to TVC’s on-site administrative assistant who will distribute the key slip as follows: UAF Facilities Services Key Shop (white and pink copy) and affiliated TVC program (yellow copy). The on-site administrative assistant will keep a log of all keys/fobs issued.
- Upon termination of employment with the University proof of key return to the FNSBSD and UAF Facilities Services key shop will be required so that the key holder will not be assessed a key replacement fee on their final check.
Access requests for TVC adjunct faculty and temporary staff:
Adjunct faculty and temporary staff who require short-term access (e.g. multi-day or week or semester-length) may request access through their affiliated TVC program/department. This may include access to TVC areas within the facility and/or outside access through the approved TVC entrance (which uses a key fob). Keys shall be issued for a specified time period and must be returned to program/department staff upon completion of the work assignment. Key use may be extended beyond the initial period (i.e. for multiple semesters) if all access and security guidelines are followed. Key requests normally take up to five days to process; please plan ahead.
Program/department coordinators requesting keys for adjunct faculty have the responsibility of monitoring access by their adjunct faculty/temporary staff and returning keys/fobs no longer used to the Hutchison principal’s office. This responsibility includes confirming at least once each semester the location/status of any electronic fobs or keys.
To request access for a TVC adjunct or temporary staff member, the program/department coordinator must do the following:
- Submit a completed UAF Facilities Key Authorization Request Form along with a Hutchison “Key Justification and Tracking Form” to the TVC Director’s Office for consideration.
- TVC-authorized key requests will be forwarded to the FNSBSD Hutchison High School Principal’s Office for consideration.
- Approved key requests: TVC’s on-site administrative assistant will contact the appropriate person/program when the key is available for pick-up through the principal’s office.
- At the time of key pick-up the employee will be required to complete and sign a FNSBSD Key Authorization Form. Please be prepared to provide a photo ID card to the FNSBSD at the time of key issue.
- FNSBSD will return the approved key request form to TVC’s on-site administrative assistant who will distribute the key slip as follows: UAF Facilities Services Key Shop (white and pink copy) and affiliated TVC program (yellow copy). - The key/fob holder will be required to show evidence of key/fob possession at specific time periods as indicated on key request form; this must be done at least once each semester.
- Upon termination of employment with the University proof of key return to the FNSBSD and UAF Facilities Services key shop will be required so that the key holder will not be assessed a key replacement fee on their final check.
Authorized Non-University access requests:
Access to TVC areas within Hutchison by those not employed by UAF or the FNSBSD (e.g. contractors, window washers, elevator maintenance personnel) must be authorized by the TVC Director. Contact the Director’s office for details.
Access Conditions:
Any person (or department/program) issued a key or key fob accepts the responsibility for promptly notifying his/her supervisor, the TVC Director’s Office, and the UAF Police Department in the event their key is lost or stolen.
